​Client Information:

At your first visit, you will be asked to complete a registration form
and provide proof of address showing that you reside in the Chetek-Weyerhaeuser School District or have a Chetek address.  
You will also be asked to provide proof for each child in your family;
for example, school records or other identifying information.
​
At each visit, you will make a number of selections from our shelves
​based on the size of your household. 
You  may visit our food shelf every 30 days to select additional items. 
A form of identification will be required at each visit.
